This is fairly common. Usually reloading alsa brings it back:

Audio channels can be muted even with the volume set above zero. Make sure you don't see MM
at the bottom of the volume bars. If you do, select and hit the <M> key

Probably not. Alsa is pretty much a catch-all for audio devices.
Perhaps alsamixer can help. Run it in a terminal with the alsamixer command. Hit F4 for capture devices.
